The LPN will play a vital role in delivering high-quality care to residents in our long-term care facility, ensuring their safety, comfort, and well-being. *Key Responsibilities: * *Resident Care:

• Provide direct nursing care to residents, including administering medications, monitoring vital signs, assisting with personal hygiene, and performing basic wound care.
  * *Care Coordination:
  
  • Assist in the development and implementation of individualized care plans in collaboration with other healthcare team members.
    * *Monitoring and Reporting:
    
    • Observe and document residents' health conditions, identify any changes or concerns, and report to the supervisor.
      * *Medication Administration:
      
      • Safely administer prescribed medications and monitor residents for any adverse reactions, maintaining accurate medication records.
        * *Team Collaboration:
        
        • Work collaboratively with other healthcare professionals to ensure cohesive and comprehensive care for residents.
          * *Documentation:
          
          • Maintain accurate and thorough documentation of all care provided, including resident observations, care activities, and changes in condition.
            * *Resident Advocacy:
            
            • Advocate for residents' needs and preferences, ensuring their dignity, privacy, and rights are upheld.
              * *Family Communication:
              
              • Communicate effectively with residents' families, providing updates on their loved ones' health status and care plans.
                * *Safety and Compliance:
                
                • Adhere to all facility policies, procedures, and regulatory standards, including infection control and safety protocols.
                  * *Emergency Response:
                  
                  • Respond to emergencies promptly, providing appropriate care and notifying the supervisor as necessary
                    * *Education:
                    
                    • Educate residents and their families about health management and care procedures as needed.*Qualifications:
                      * *Education:
                      
                      • Graduate of an accredited LPN/LVN program.
                        * *Licensure:
                        
                        • Current and valid Licensed Practical Nurse (LPN) license in Wisconsin or compact license.
                          * *Experience:
                          
                          • Minimum of 1 year of experience in long-term care or a similar healthcare setting preferred.
                            * *Certifications:
                            
                            • Basic Life Support (BLS) certification required.
                              * *Skills:
                              
                              • Strong clinical skills, excellent communication and interpersonal skills, ability to work effectively in a team, attention to detail, and a compassionate approach to care.
                                * *Knowledge:
                                
                                • Understanding of long-term care regulations, infection control procedures, and best practices in geriatric care.*Shift:
