*Job Overview:*_We are seeking a dedicated and compassionate Long-Term Care Licensed Practical Nurse (LPN) to join our team. The LPN will play a vital role in delivering high-quality care to residents in our long-term care facility, ensuring their safety, comfort, and well-being. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for working with elderly patients._*Key Responsibilities:** *Resident Care:* Provide direct nursing care to residents, including administering medications, monitoring vital signs, assisting with personal hygiene, and performing basic wound care.* *Care Coordination:* Assist in the development and implementation of individualized care plans in collaboration with other healthcare team members.* *Monitoring and Reporting:* Observe and document residents' health conditions, identify any changes or concerns, and report to the supervisor.* *Medication Administration:* Safely administer prescribed medications and monitor residents for any adverse reactions, maintaining accurate medication records.* *Team Collaboration:* Work collaboratively with other healthcare professionals to ensure cohesive and comprehensive care for residents.* *Documentation:* Maintain accurate and thorough documentation of all care provided, including resident observations, care activities, and changes in condition.* *Resident Advocacy:* Advocate for residents' needs and preferences, ensuring their dignity, privacy, and rights are upheld.* *Family Communication:* Communicate effectively with residents' families, providing updates on their loved ones' health status and care plans.* *Safety and Compliance:* Adhere to all facility policies, procedures, and regulatory standards, including infection control and safety protocols.* *Emergency Response:* Respond to emergencies promptly, providing appropriate care and notifying the supervisor as necessary* *Education:* Educate residents and their families about health management and care procedures as needed.*Qualifications:** *Education:* Graduate of an accredited LPN/LVN program.* *Licensure:* Current and valid Licensed Practical Nurse (LPN) license in Wisconsin or compact license.* *Experience:* Minimum of 1 year of experience in long-term care or a similar healthcare setting preferred.* *Certifications:* Basic Life Support (BLS) certification required.* *Skills:* Strong clinical skills, excellent communication and interpersonal skills, ability to work effectively in a team, attention to detail, and a compassionate approach to care.* *Knowledge:* Understanding of long-term care regulations, infection control procedures, and best practices in geriatric care.*Shift:*Nights (40 Hrs. per Week)*Duration:*8 - 12 Weeks*Pay Breakdown:*$42.00/Hr.
