Job Description:

Encompass Health is seeking a dedicated Licensed Practical Nurse (LPN) to join our team, focused on providing top-notch rehabilitation and home health/hospice services. In this role, you will work closely with an interdisciplinary team dedicated to delivering high-quality care that helps patients regain independence after serious illness or injury. Your primary responsibilities will include patient care, monitoring health conditions, administering medication, and assisting with daily living activities. Provide direct nursing care according to the prescribed patient treatment plan. Monitor patient health and document their medical history. Administer medications and maintain accurate records of treatments. Assist with patient mobility and physical therapies. Educate patients and families on health care needs, conditions, and options. Ensure compliance with all health care regulations and standards. Strong practical nursing skills. Excellent patient management abilities. Proficient in using medical software and electronic health records. Ability to work effectively within an interdisciplinary team. Strong communication and organizational skills. Medical Insurance Dental Insurance Vision Insurance 401k Valid LPN license in the state of practice. CPR/BLS certification.
