We are looking for Per Diem 1099 Independent Contractors to go into members' homes and provide Health Plan Wellness Assessments, educate members, and provide advice and emotional support that drives demonstrated improvements in wellness outcomes. Assessment types may include care coordination, fall prevention and care transitions programs to reduce hospital readmissions. As a Registered Nurse, in an ever-growing organization, you will be presented the opportunity to perform clinical assessments to members of varying ages and to utilize your specific skill set to offer awareness and member education. Requirements for this role include:

• A current and unrestricted license as a Registered Nurse.
• 2 years' experience as a Registered Nurse.
• Must be able to visit members in their private residence.
• A valid state driver's license.
• Proof of adequate automobile insurance coverage for the state of residence.
• Must have a laptop computer in order to complete the Assessment.
• Must have good computer skills as we do all charting electronically on a laptop or tablet.
