*Now Hiring RNs to make skilled patient visits in Fox Valley Area -*_Great Lakes Home Health Care is looking for an experienced Home Health RN to join our team. We are a startup agency who are excited for the future of the home care industry and are very passionate about giving the very best care to improve patient lives. We just passed the State of Wisconsin Survey with *ZERO DEFICIANCIES* and are looking for amazing RNs to start seeing patients so that we can become Medicare certified._The *Registered Nurse (RN) *provides and directs provisions of nursing care to patients in their homes in collaboration with the Primary Care Physician and in compliance with applicable laws, regulations, and agency policies. Also, The RN Registered Nurse job coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient and family.*License Requirements** Must have current RN licensure in state of practice.* Current drivers license, vehicle insurance and access to a dependable vehicle or public transportation.* Current CPR certification required.*Qualifications: ** Provide care to patients as outlined by the Plan of Care.* Assist in the orientation of new staff as needed.* Does Supervisory Visits as needed.* Prepares clinical notes daily.* Reports to the Agency Director or Designee on a regular basis regarding the general, physical and mental condition of the patients.* Administers prescribed medication and treatment in accordance with the physician's orders.* Participates in in service education programs, patient care conferences and staff meetings as scheduled.* Follows company policies and procedures.* All other duties as assigned.*Other Requirements* - 2 years of Home Health Experience - Knowledge of filling out an Oasis C or DJob Types: Part-time, PRNBenefits:* Flexible scheduleMedical Specialty:* Home HealthSchedule:* Monday to Friday* Rotating shift* Weekend availabilityCOVID-19 considerations:We are providing: N-95 masks, Hand Sanitizer, Fluid Resistance Gowns, Gloves, Face Shields/Goggles.
