In-Home Safety Services Community Worker / Truancy
LiaisonProfessional Services Group, Inc.Description:
The In-Home Safety Services Community Worker works with families
referred from the Winnebago County Child Protective Services units
to maintain safety in homes where children are at risk for removal.
In addition, this position provides services to youth who have been
re-unified with their family after being placed in residential
treatment, foster care or treatment foster care. The position also
provides services to youth and families involved with Youth Justice
services.
As a Truancy Liaison, you will serve as a liaison to middle and
high school aged youth who are at-risk for truancy, academic
failure, delinquency, gang involvement, and alcohol and drug
abuse.
SALARY: $18.00 an hour. Plus a $1,000 sign-on bonus!
ESSENTIAL FUNCTIONS:
Interview and evaluate clients to complete intake process.
Evaluate youth and families for treatment needs and refer to
appropriate resources.
Analyze client situations, capabilities, and problems to determine
services required to control safety threats.
Formulate, implement, evaluate and review safety assessments and
plans in eWiSACWIS.
Exercise judgment to intervene as an advocate for clients in
emergency and crisis situations.
Act as an on-call resource to evaluate and determine necessary
responses in de-escalating crisis situations with youth and
families.
Conduct face-to-face contact with families, per program standards,
to ensure safety and cooperation with safety plan.
Complete all required paperwork/documentation, per program
standards.
Attend all required staffings.
Provide timely client updates to other involved
employees/agencies.
Formulate, implement, evaluate and review individual client
treatment plans; including support and assistance with school
assignments.
Monitor client behavior and school achievement by conducting
in-school checks minimally on a weekly basis.
Provide assistance to school personnel and students as determined
by Winnebago County worker and program supervisor and report client
behavior and achievements.
Update parents/guardians on client progress and provide problem
solving interactions.
Prepare reports and discharge summaries per program standards.
Provide supervision and support to youth during school breaks and
throughout summer programming.
Ability to operate a motor vehicle safely and obey all traffic
laws.
Other duties as assigned.
Requirements:
Must have reliable transportation, on a daily basis, a valid
driver's license and automobile insurance.
Education and Training:
Must have a Bachelor's Degree in Social Work, Psychology, Criminal
Justice, or other related field.
Must be CPR certified.
Experience:
One year experience working with at-risk youth and families
preferred.
Knowledge, Skills and Abilities:
Ability to read, speak and write clearly in English.
Ability to exercise good judgment and discretion in establishing
work schedule.
Must be able to work cooperatively and collaboratively with the
Winnebago Department of Human Services.
Must establish and maintain productive and professional
relationships with clients, colleagues, the school district, and
other community agencies.
Must maintain client confidentiality.
Ability to prioritize and make appropriate decisions.
Ability to work independently and effectively manage time.
Must possess strong oral and written communication skills.
Ability to provide culturally, ethnically and gender sensitive
services.
Ability to follow directions and implement recommendations.
Possess creativity in identifying problems and solutions.
Ability to self motivate and remain on task.
Knowledge and compliance with policies and procedures as detailed
in the employee guide and manual.
Work Environment:
This position operates in a professional office environment as well
as within the community. This position is required to go into
client homes, schools, summer employment worksites and any other
setting necessary to provide services. This position routinely uses
standard office equipment such as computers with various software
programs, phones, photocopiers, shredders, filing cabinets and fax
machines. Exposure to environmental conditions such as air
conditioning, dust, odors, and outdoor weather conditions occur.
Exposure to household pets may occur.
Physical Requirements:
While performing the duties of this position, the employee is
regularly required to: converse and communicate with others;
possess visual acuity which allows employee to view a computer
monitor, read documents and safely operate a motor vehicle; remain
in a stationary position as well as move about inside schools or
other community settings (i.e. client homes, county buildings);
ascend and descend stairs; operate a computer and other office
technology; and move objects weighing up to 20 pounds.
Travel:
Travel is primarily local during the business day.
Client Caseload and Hours of Work:
This position's caseload will vary based on the needs of the
program. The hours of work for this position are varied. This
position is expected to work some evening and weekend hours to
accommodate the needs of the program. Position must be available
via cell phone 24/7, unless other coverage arrangements have been
made.
Attendance:
This position requires regular and reliable punctuality and
attendance.
